Before Your Appointment

Please log in to the Patient Portal and complete the following prior to your visit:

1. New Patient Questionnaire and Forms
   Please complete all required patient forms and questionnaires through the
   Patient Portal before your appointment.
2. Privacy Policy
   Review our privacy policy in the portal and keep a copy for your records.
3. Office and Financial Policies
   Please review and sign these policies electronically in the Patient Portal.

Completing these steps in advance will help reduce waiting time and allow our staff to focus on your care during the visit.
